I-9 Employment Verification Forms

An I-9 form (official name: Form I-9, Employment Eligibility Verification) is the U.S. form employers use to confirm that a new hire is:

*Who they say they are (identity), and

*Allowed to work in the United States (work authorization)

Who uses it

Employers must have most employees complete it when they start a job (it’s required by federal law).

Employees fill out part of it and show acceptable documents.

The employer (or an authorized representative) reviews the documents and completes the employer section.

Document options (choose one path)

Option A: One document from List A

A List A document proves both identity and work authorization (you only need ONE).
Examples:

U.S. Passport or U.S. Passport Card

Permanent Resident Card (Green Card)

Employment Authorization Document (EAD)

Option B: One document from List B + one from List C

If you don’t have a List A document, bring:

List B (identity), PLUS List C (work authorization)

Common List B examples:

State driver’s license or state ID card

School ID with photo

Voter registration card (some employers accept—depends on their policy)

Common List C examples:

Social Security card (not restricted)

Birth certificate (issued by a U.S. state/territory)

Important notes

Documents must be original and unexpired (unless the I-9 rules specifically allow otherwise for a particular document type).

Names should match your hiring paperwork. If they don’t, bring supporting documentation (e.g., marriage certificate/court order), if your employer requests it.

The employer decides which acceptable document(s) you present—you choose from the approved lists.
